10th Class or Equivalent Examination “Must have First Aid Certificate” from Govt. approved Institutions

Purely on Merit and follow the Rule of Reservation.

Total Marks – 100
I) 75 Marks obtain in qualifying examination
II) Weight age for number of years since passing qualifying examination is upto 10 marks @ 1
Marks for each completed year.
III) 15 marks weight age will be given to the candidates working earlier in the organization approved by the Government. Who are presently working in (HDS/CDS/Arogyasree HCT, trauma care,APCOS,NHM and other GOI schemes who are appointed on Contract / outsourcing basis through District Selection Committee are appropriate selection process which ever is applicable under the control of Director of Public Health and Family Welfare/ Commissioner APVVP / DME only as per GO.Ms.No.301 HM&FW (B1) Dept, dt.20.06.2020.
a) 2.5 Marks per Six months in Tribal Area
b) 2.0 Marks per Six months in Rural Area
c) 1 Marks per Six months in Urban Area.
IV) Weight age for candidates who are working on COVID DUTIES in Govt. COVID HOSPIALS appointed by the Dist. Selection Committee as per GO.Rt No.211, HM&FW (B2) Dept. dt.08.05.2021.
a) 5 Marks per Six months
b) 10 Marks per one year
c) 15 Marks per One year Six months.
IV) There will be no Interview Marks.

How To Apply Process :

The Applicants can download the Application & the details of vacancies available & eligibility particulars through Website address spsnellore.ap.gov.in/notice/recruitment.

Filled Applications shall be submitted in the O/o District Medical & Health Officer, Nellore in all working days from 22.11.2021 to 05.12.2021 at 10.30 AM to 05.00 PM along with all required certificates and requisite application fee by way of Demand Draft drawn in favour of DM&HO, Nellore payable at Nellore, on or before 05.12.2021.

Last date for submission of filled applications is 05.12.2021 at 5.00 PM.

Guidelines and Instructions for filling up of application:

The filled in application should be submitted in person duly enclosing the following certificates along with application fee and the application form at O/o DISTRICT MEDICAL & HEALTH OFFICER, NELLORE from 22.11.2021 to 05.12.2021 by 5 PM on working days in working hours i.e 10.30 AM to 5.00 PM. The application without signature of the applicant or without any of the following enclosures will be summarily rejected.

1. S.S.C or Equivalent examination Marks Memo.
2. Intermediate or 10+2 examination Marks Memo.
3. Qualifying Examination Pass Certificate.
4. Marks memos of all the years (qualifying examination)
5. Registration and Renewal certificates of respective councils.
6. Internship Certificate if any applicable.
7. Latest Caste certificate issued by the Tahsildhar / MRO concerned
8. Study certificate for the years from 4th class to 10th Class. In case of Private study residence certificate from the Tashildhar /MRO concerned for the above period (4th to 10th Class study period).
9. PH certificate (SADAREM CERTIFICATE) in respect of candidates Claiming reservation under PH Quota
10. Sports certificate in respect of candidates claiming under Sports quota.
11. Relevant Certificates in respect of candidates claiming Ex Service man Quota
12. 1 photograph duly pasted on the application form with self attestation
13. Copy of appointment order and Service Certificate for in-service candidates who are working on contract service with Government under Medical and Health Depart should produce service certificate from concerned officer duly counter signed by the concerned HOD.

Rs. 800/- for OC Candidates,
Rs. 500/- for SC / ST /BC Candidates
The application fees should be paid by the way of Demand Draft from any Nationalized Bank, in favour of DM&HO, Nellore payable at Nellore.
